Death Certificate Application Form
You can obtain a copy death certificate for a death that was registered in Poole by downloading and completing this application form, for example if you have lost your original death certificate.
The cost of all standard death certificates is £7.00. We cannot issue a certificate until we have received payment. Cheques or postal orders should be made payable to Borough of Poole.
