Assessment of Care Needs Application
If you are an adult resident of Poole and think you could benefit from services to assist you to live independently in safety and comfort, you can request a Community Care Assessment using this form, or ask someone else to do so on your behalf (such as a friend, relative, GP or District Nurse). The application form is available in two forms:
- a pdf version to print off, complete and return to us in the post, or
- a Word version to download, type into, save and email back to us at: sshelpdesk@poole.gov.uk
We aim to:
- Reply to your request for an assessment within 2 days of receiving your form. Emergency requests may be dealt with sooner than this.
- Carry out your assessment within one month of receiving the request, although there may be circumstances where it may take longer than this.
