Home Education Form

Please note this form is not an application form. This form is for parents to give the LEA some basic information regarding their decision to educate their child at home. If parents wish to provide information in an alternative way please feel free to do so. Once the information has been received, arrangements will be made for the Monitoring Officer to visit at a time convenient to all.
