Early Years - 3 and 4 Year Old Funding Headcount Form

This form is intended for Managers of Early Years Childcare settings who wish to claim Three and Four Year Old Funding for a child or children in their setting.

This form is not a stand alone document and needs to be included with an Actual Return Form and a Parent Declaration Form.
