People Moving On From Hospital Living Project
143 people with Learning Disabilities living in NHS hospital accommodation across Dorset are the focus of this project. They are being helped to move on to a home of their choice in line with the Government guidelines and principles in the White Papers
- Valuing People a New Strategy for Learning Disability for the 21st Century
- Our Health, Our Care, Our Say
These individuals, with input from family and others who support them, will develop a plan which will help them to be:
- in control of their lives
- supported to remain safe in their new homes.
